PHARMACY BENEFIT INFORMATION
Effective July 1, 2011, active employees AND retirees under 65 have pharmacy benefits through Envision Rx.
The EnvisionRxOptions Help Desk can be reached online, or at 1-800-361-4542. Please use this number or visit the Envision website for:
- Requesting a new/corrected ID card.
- Finding a participating network pharmacy in your area.
- Questions about formulary/non-formulary medications.
- General questions regarding pharmacy benefits and co-pays.
Please note that Envision Pharmacy ID cards will ONLY list the name of the primary insured (employee or retiree). If you need additional cards for your spouse or dependent child, you can simply photo-copy your Envision Pharmacy card for their use. The group and ID numbers for you and your covered dependents are the same. Be aware that effective 7/1/11, you must use your Envision ID card at the pharmacy, NOT your Anthem Blue Cross card.

MAIL ORDER REFILLS
Envision partners with Costco Pharmacy for mail order prescriptions. You do not need to be a Costco member to utilize this benefit. Existing mail order prescriptions which have refills remaining will automatically transfer. However, for certain narcotic medications and for prescriptions that have expired, you will need to obtain new prescriptions from your physician for a 90-day supply. You will need to contact Costco to set up a user profile and make payment arrangements.

You have 2 options for using this service. 1) Traditional mail order service – where all orders are placed through the mail or by phone. 2) Online order service – where orders can be placed online at www.pharmacy.costco.com.

SPECIAL INFORMATION FOR RETIREES OVER 65
There is no change to the pharmacy benefit for Medicare-eligible retirees over age 65. Your prescription benefits are administered by Express Scripts.
